Coaching Agreement
In the event that we decide to work together, I have a very simple coaching agreement I ask my clients to sign. I provide it on my site so that you can review it at any time.
Life Coaching Contract
I hereby attest that I am engaging Jessica Woodard as a life coach for the purpose of making positive changes in my life.
I acknowledge that the following practice policies have been explained to me, and that I am in agreement with them.
Life coaching is a goal oriented practice that relies on the client’s (my) commitment and follow through. I am dedicated to establishing and working towards my goals.
Sessions with my life coach require preparation and commitment from both parties. All sessions will be paid at the time of booking.
Should I cancel with less than 24 hours notice, my fee for the session will be forfeit unless I have an emergency. In the case of an emergency, or in the case of more than 24 hours notice, my session may be rescheduled at no additional cost.
Should my coach need to cancel, she will cancel at least 24 hours in advance, barring extreme emergencies. Should my coach cancel she will reschedule our session as soon as possible.
Email and text are both acceptable methods of communication for scheduling.
The coaching relationship may be dissolved by either party.
Should I choose to dissolve the coaching relationship after one or more sessions purchased as a package, the purchased sessions will be charged at the a la carte rate and the remainder of the package fee will be returned.
Should my coach choose to dissolve the relationship after one or more sessions purchased as a package, the purchased sessions will be charged at the discounted package rate and the remainder of the package fee will be returned.
Should either party choose to dissolve the relationship before any sessions (and more than 24 hours in advance of the first session) the full package fee will be returned.
While life coaching does not have legal confidentiality mandates, it is expected that my coach will maintain confidentiality regarding my work with her, with the following exceptions:
If my coach believes I am at risk of harming myself or others, she will file a report for my own well being and the well being of those around me.
Life coaching, while often having a positive effect on mental health due to improved life outcomes, is NOT a therapeutic mental health practice. My coach has not represented herself as a therapist or counselor, and I am not engaging her to diagnose or treat me for any mental health issues.
